POSITION SYNOPSIS AND PURPOSE: 

 

Responsible and reporting to the Director of Care, the Registered Nurse Supervisor is responsible for assessing, planning, implementing and evaluating the nursing care of the residents.  Prepares and administers medications as ordered and according to the Standards of Nursing Practice as set by the College of Nurses.  Responsible for and administers Controlled Drugs and Narcotics used in the Home.  Consults physicians and external resources as needed in his/her professional judgement.  Supervises, trains and delegates duties to the Registered Practical Nurses (RPNs) and Personal Support Workers (PSWs).  Maintains the smooth operation of each resident home area as designated by assignment and shift.  Promotes the mission, vision and values of Valleyview Home.

 

MAJOR RESPONSIBILITES AND ACCOUNTABILITIES: 

 

Direct Nursing Care - 30%

  • Prioritizes emergent issues and provides direction and support to RPN and PSW staff concerning resident care, medication administration, resident social issues and resident home area operations.
  • Assesses the physical, emotional and psychological well-being of residents through observation and analysis of test and physical health indicators to evaluate the care plan and identify modifications which may be required.  Ensures care provided as per each resident care plan and ensure continuity of care.
  • Provides nursing care including measuring and recording of resident vital signs, wound assessments, blood work, weight and neurological assessments using appropriate methods and equipment.
  • Ensures proper storage of medications including the safe destruction of controlled substances.
  • Directs care and notification in the event of resident death.
  • Accompanies physician on rounds providing information on resident condition and progress, advocacy and assists with examinations ie. positioning, dressing and undressing residents.  Transcribes verbal or written physician’s orders for medication or other treatment.
  • Prepares and administers resident’s medication in accordance with physician’s orders and the standards of practice from the Ontario College of Nurses including narcotic and controlled drugs.
  • Monitors and participates in the personal care of residents/clients, as required; for example hygiene, feeding, positioning, transferring,  ambulation, toileting in accordance with Ministry standards and Valleyview policy and procedures.
  • Practices sterile techniques and other infection control procedures to minimize the transmission of infection.
  • Prepares and applies clean and sterile dressings and bandages and replaces as required.  Cleanses wounds and lesions.  Removes stitches and clips.  Implements appropriate tools as legislated for monitoring and ensures protocols are adhered to. Performs delegated Medical Acts (duties), ie. vaccinations, inoculation, eye irrigations, death pronouncements etc.
  • Administers first aid and emergency health measures such as CPR as warranted.
  • Performs other duties, responsibilities and functions as assigned.

Planning and Implementing - 15%

  • Administers and interprets results of assessments such as mental status, fall risk, pressure ulcer risk, wound care, continence etc. and interpretation of lab results as a component of the overall nursing assessment of resident health status and care planning. Contacts physicians as needed and processes any orders received.  Chairs admission and annual care conferences and unit meetings.  Works collaboratively with physicians, residents and families to ensure resident care needs are met. Updates, evaluates and modifies care plans for residents using RAI-MDS within the required timelines. Charts using computer-based programs (eMar, PCC, assessment apps).  Maintains inventory ensuring medications are properly stored, safe and secure.
  • Liaises, refers and advocates with community resources to coordinate admittances, transfers and discharges/deaths, hospitals (acute care, rehabilitation and palliation), public health department, home care program, long term care facilities, ambulance services, diagnostic services, the coroner’s office and social services.
  • Counsels, instructs and confers with resident/client and family in planning care; provides emotional support; responds to enquiries and resolves complaints.
  • Ensures compliance with programs, regulations and guidelines set out by the Fixing Long-Term Care Act, the Ministry of Long-Term Care, Ministry of Labour, etc.
  • Participates in Quality Improvement and Risk Management activities.
  • Prepares and maintains documentation such as care plans, health care records, medication administration and narcotic record, nursing records, statistics and reports.

Teaching and Learning - 10%

  • Oversees the training and orientation of any new RN/RPN staff and students.
  • Oversees the training of new PSW staff and students.
  • Provides support and health teaching to residents and family members.
  • Updates and explains new initiatives provided by pharmacy, the Ministry of LTC, etc. to staff and residents as required.
  • Participates in committees within the Home (ie. Lift Committee, Palliative Care Committee, Wound Care).
  • Ensures obligations for reflective practice are met as per CNO requirements.  Ensures ongoing recertification of CPR, mask fit testing, lift training and other annual staff education requirements are complete.

Administrative - 15%

  • Oversees staff scheduling in off hours.
  • Oversees home operations in off-hours, including building safety and initiating emergency response.
  • Executes Resident Abuse Legislation after hours in the absence of management including Mandatory Reporting, if applicable.
  • Takes water and fridge temperatures and reports appropriately.
  • Answers administrative-based phone calls after hours.

Supervisory Role - 30%

  • Supervises PSW staff, RPN staff and students working within the nursing department.  This is ongoing throughout the duration of every shift.
  • Indirect oversight of other departments after hours including contract cleaners, environmental services, dietary, activation, PanBio clinic, Security.
  • Responds to Ministry of Long-Term Care involvement including family-based complaints and findings.
  • Provides conflict management between residents, family members or staff.
  • Ensures infection prevention and control measures are met when called to PanBio clinic for direct staff and family testing.
  • Each RN is required to carry an internal phone and may be called at any time to attend to situations (including nursing breaks).

Note: The duties and responsibilities outlined above are representative but not all-inclusive.

All activities are expected to be performed in a safe manner in accordance with the Occupational Health and Safety Act and its Regulations along with Corporate Safety policies, procedures and programs.  In addition, all necessary personal protective equipment must be used and maintained in good condition.

This position must be compliant with all provisions of the Occupational Health and Safety Act, related to “Duties of a Supervisor” and "Duties of a Worker".

 

QUALIFICATIONS

 

Education/Experience:

  • Graduate of an RN program from a recognized institution
  • Must have current registration with the College of Nurses of Ontario without restriction.
  • Current CPR Certification
  • A minimum of two (2) years’ experience in geriatrics, rehabilitation or long-term care nursing is required
  • Experience supervising other staff and providing direction to same as well as evaluating work performance within a unionized environment

Knowledge/Skill Ability: 

  • Must have the ability to direct nursing care provision to ambulant and non-ambulant residents and clients with a focus on restoration, health promotion and palliation regardless of complexity and predictability autonomously within his/her scope of competence.
  • Demonstrated knowledge and ability in the practical application of nursing theory, biological and social sciences, principles of gerontology and accepted best practices to the standard established by provincial and nursing legislation.
  • Must have strong organizational abilities with demonstrated ability to prioritize and direct other staff.
  • Excellent verbal and written communications skills, conflict resolution and leadership.
  • Demonstrated strong computer skills and be familiar with Point Click Care software.
  • Must be reliable, accountable and flexible in scheduling to respond to the needs of the facility.

 

CONDITIONS OF EMPLOYMENT: 

  • Successful completion of a police record check with vulnerable sector clearance. 
  • Satisfactory TB documentation.
  • Must meet all applicable provincial directives with respect to COVID-19 immunization/testing.

Remuneration Range: $46.5650 - $56.60 per hour (2024 rates).
